Home for All Continuum of Care 2023-2028 Strategic Plan
The Home for All Continuum of Care 2023-2028 strategic plan builds on the collective work of past and current leaders who have laid the foundation for the success of our Continuum of Care. From case managers to executive directors, our system is composed of people who have committed their lives to addressing and ending homelessness.
About the Home for All Continuum of Care
Continuum of Care is a network of providers and organizations that work in coordination to address the needs of those experiencing homelessness. Home for All’s staff is housed within the Heart of Illinois United Way and provides strategic leadership to this network.
Home for All Vision
End homelessness in Peoria, Tazewell, Woodford, and Fulton Counties by ensuring access to permanent, safe, and affordable housing.
Strategic Plan Goal
Create a response system that aims to prevent and quickly resolve homelessness through braiding housing opportunities, support services, and resources to address the multiple dimensions of housing insecurity.
Guiding Principles:
- Belief that ending homelessness is possible
- Focus on addressing inequities
- Incorporation of lived expertise at every level
- Commitment to the use of data to inform decision-making
- Invitation to all who want to join the work
Strategic Plan Engagement Process and Stakeholders
Home for All leadership worked to engage stakeholders across the service area to inform the direction and key elements of the strategic plan. Nearly 100 unique stakeholders and organizations helped shape Home for All’s strategic direction for the next five years.
Priorities:
- Build awareness and community engagement
- End youth and veteran homelessness
- Create new housing opportunities
- Improve performance of existing system
- Strengthen CoC infrastructure
