All funded housing providers within the Continuum of Care work from one list of individuals and families experiencing homelessness. Those on the list are prioritized for resources based upon needs. In order to be placed on the list, individuals and families go through a process called “Coordinated Entry” during which time they are asked questions to understand their current situation and assess their needs. If you need to be placed on the Coordinated Entry list, you can visit:

Coordinated Entry is a system that targets a limited pool of housing resources toward people who are chronically homeless and/or are at advanced risk in their homelessness because of disability or violence.
*Opportunities for Adults with a Disabling Condition and Seniors
There are income-based housing opportunities targeted to people age 18+ with a disabling condition that can be verified by a doctor; as well as those who are a certain age (often 55+ or 62+), regardless of presence of a disability.
Advocates for Access maintains lists of these complexes on their website and can provide guidance on the application process.
